How can I get support?

You can access the free support from our service via telephone, face to face or online

An assessment will normally take about 45 minutes, which allows us to discuss and understand what you would like help with and discuss the right course of treatment.

We are a primary care service and therefore not able to deal with crisis situations. If you need to speak to someone urgently please contact your local Accident and Emergency Department or GP.

Professional/supported referral

If you are a professional and want to support some to make a referral to our service, you are welcome to complete this form on their behalf. Please make it clear in “referral reason” who is making the referral and that you have explicit consent from the client to make this referral.

Once we have received your completed form we will email you to give you the option of completing your assessment yourself online. If you chose to complete an online assessment, we will give you a short call back to review your answers and discuss treatment options.

If you do not want to do your assessment this way, we can book you for a full telephone or face to face assessment.

Contact us on 01924 234 860 any time between 8.00am and 8.00pm  Monday to Thursday, Friday 8.00pm-4.00pm, 9.00am and 4.00pm on Saturday, 10.00am and 4.30pm on Sunday. Please note for our last assessments on the day will start 1 hours before closing time.

What will happen once I have referred myself?

The details of everyone who refers to our service are stored on our confidential database, which shares anonymous statistical data with the department of health. for full information about our data sharing and storage please see our policies and procedures.

If we have a clinician available, and you are free to complete a 45 minute assessment, we will complete the screening when you first contact us, over the phone or face to face. However if this is not possible, we will offer you an alternative time within the next 48 hours. If at any point during your first contact and assessment we think a different service would be more suitable for your needs, we will discuss this and to help you with the next steps.

You can also complete an online self assessment. If you complete the online self referral form and leave us your email address, we will send you the form to complete in the comfort of your own home. A Clinician will then call you to discuss this and arrange next steps for treatment.

The purpose of this appointment is to make sure that we are the right service for you and to help decide which of our many treatment options will be best for you. If we think we may not be the best people to help you, we can help recommend or refer to a more appropriate service.

We will complete a questionnaire with you to help you further assess your difficulties and monitor any change. You can find out more about why we complete these here or have a look at the QUESTIONNAIRES.

This questionnaire is completed at every appointment with us. For further details about why collecting this data is important please see the data collection section of this website in our policies and procedures.

At the end of this appointment, if we feel our service is the right one to help you, we will discuss a treatment plan with you and book you in to start your journey straight away.
